For post-acute (short-stay) residents:

For chronic care (longer than 30 day stay) residents:

• Depressed or Anxious Worsening
• Incontinence (Low Risk)
• Indwelling Catheters
• Mobility Decline

• Physical Restraints
• Pressure Sores (High Risk)
• Pressure Sores (Low Risk)
• Urinary Tract Infections
