How do you find your facility's quality measure scores on Nursing Home Compare?

Step 1 – Go to www.medicare.gov
Step 2 – Click on "Nursing Homes"
Step 3 – Click on "Nursing Home Compare"
Step 4 – Click on "New Search"
Step 5 – Select Wisconsin under "Select a Geographic Area," then click "Next Step"
Step 6 – Type in part or all of your facility's name under "Nursing Home Name Search"
Step 7 – Check the box to the left of the facility you want to select, then click "Next Step"
Step 8 – You can choose to view additional information by clicking on any of the following topics:

• "About the Nursing Home"
• "Quality Measures"
• "Inspection Results"
• "Nursing Staff"
• "View all information for this nursing home"
