 Health Care-Acquired Conditions in Nursing Homes

There is no longer any question: adopting safer processes for delivering health care can save lives and lower costs. Improving health care quality is an urgent priority that requires everyone’s efforts. MetaStar is committed to collaboration and coordination so we can all maximize impact for Wisconsin and make wise use of resources.

Focusing on improving safety for every resident, nursing homes that join in the MetaStar’s health care-acquired conditions (HAC) initiatives will contribute to a 40% national reduction in HAC rates. Work to reduce HACs begins with technical assistance to improve pressure ulcer prevention and reduce physical restraint use, building on the success of nursing homes that participated in recent initiatives on these topics.

Facilities can expect to receive:

  • free onsite consultation
  • training in quality improvement skills and strategies
  • evidence-based tools and resources to enhance existing efforts
  • ongoing education 

We can help augment relationships that will strengthen team dynamics and highlight the value of each team member’s role in quality improvement.

Join our new cross-setting community coalition! There is no membership fee, and members are working together to reduce pressure ulcer and physical restraint rates by 40% in Wisconsin. Beginning later this year, MetaStar will launch a statewide Learning and Action Network (LAN) for nursing homes that will address catheter-associated urinary tract infections, falls and other HACs. Participants will be able to learn from local and national peers, obtain tools and resources, and participate in improvement collaboratives.

By improving processes of care, facilities can reduce waste and increase staff satisfaction while building a framework that will prevent adverse outcomes. MetaStar is a an independent, objective and trusted partner that helps nursing homes connect with the largest federal program dedicated to improving health quality at the local level. During the past three years, Wisconsin nursing homes participating in our projects saw a relative improvement rate of 70.75% for physical restraints and 62.8% for pressure ulcers.
